Behavioral Health Educator

Behavioral Health Educator will provide guidance and coordination of care to clients living with HIV/AIDS with complex substance abuse and mental health conditions. They will develop effective interpersonal relationships with clients and work collaboratively with the interdisciplinary care team to improve health outcomes. They will utilize internal and community resources to educate clients and form a care plan with specific health outcomes.

Care Coordinator

Coordinating care and providing guidance to clients with complex psychiatric and/or co-morbid medical conditions. Develops effective interpersonal relationships with clients and works collaboratively with the interdisciplinary care team to improve health outcomes. Utilizes internal and community resources to educate clients and form a care plan with specific health outcomes. Works collaboratively with Care Manager to ensure thorough and timely monitoring of approximately 50 clients and their progress toward Care Plan goals. Reviews Care Plan with Care Manager and confirms intensity level of identified client to ensure service needs are met. Interacts with client and client caregivers to ensure continuity of care, patient adherence to care plans, and identifications of barriers preventing adherence to care plan. Bachelor’s Degree in health or human services field required and one-year working with an underserved population through employment, volunteer work, and internships; Experience with chronic disease including HIV/AIDS; Substance user; Mentally Ill; and Lesbian, Gay Bisexual and Transgender (LGBT). Fluency in a second language preferred.

Navigation Coordinator

Navigation Coordinator provides all home-based health promotion, adherence education, support and skills building services to HIV/AIDS patients on a quarterly, monthly or weekly basis. Helps patients stay connected to care; links patients to community resources; provides escort services, identifies and helps remove barriers to care.

Social Worker

The Social Worker will be an integral part of the HIV Patient Navigation program that will increase the number and proportion of PrEP/PEP- eligible MSM of color and transgender persons who are aware of PrEP/PEP, who are screened for PrEP/PEP eligibility, who receive Brief Intervention and PrEP/PEP education and are linked to a PrEP/PEP provider. These services will be provided at an STC clinic, either in Manhattan or the Bronx, or in the field. The Social Worker must have current LMSW or LCSW license. Must have at least three years’ experience working in the field of HIV/AIDS managing services for the LGBTQ population. . They must possess an understanding of community level work and the importance of collaborating and coordinating with other organizations; and effective communication and documentation skills. Fluency in Spanish a plus.

Photography Intern

The Photography Intern will support the work of the Director of Photography and the Executive Team of ASCNYC in New York City. You can be a part of the team which will be launching ASCNYC’s 25th Anniversary campaign with multiple donor cultivation activities and our signature fundraising events:  New York's Best in Drag (spring) and the Positive Changemakers Gala (fall).
